In the late 1950's Dr. A.G. Jacobs, a practicing dentist and founder of Safe-T-Gard, began producing mouthguards using denture lining silicone materials. In the early 1960's Dr. Jacobs pioneered boiling water mouthguards and was granted three patents for his innovations. In addition Safe-T-Gard was the innovator of mouthguards in colors. Dental Warranty & Indemnity certificate
Important: To be covered against Dental Injury, you must keep this certificate. This certifies that SAFETGARD CORP. of Golden, Colorado agrees to indemnity the user of this mouth guard for a period of one year from the date of its purchase against any expenditure in any amount not to exceed $8000.00 (excess of any valid or collectible insurance ) incurred for professional dental services arriving out of injury or damage to the user’s sound natural teeth. If such damage is incurred while participating in a supervised athletic contest or practice session in connection therewith while wearing the SAFE T GARD protective mouthpiece in accordance with instructions, subject, however to the following conditions.
1. Payment is limited to $75.00 per injured tooth and $150.00 per replaced tooth. The total amount of payment cannot exceed $8000.00
2. That written noticed of injury and verification from the user and the supervising athletic official accompanied by this certificate must be given to SAFE T GARD CORP. within 30 days after the day of injury.
3. Treatment must be provided by a licensed dentist within 30 days after the day of injury.
4. This certificate does NOT provide indemnity for:
A. Orthodontic treatment.
B. Preparations of a tooth to be used as an anchor for a bridge.
5. No payment shall be made under this certificate of indemnity after a period of 1 year from the date written notice of the injury is given.
